Heart disease is the leading cause of death in the United States — and high cholesterol is one of its most significant and modifiable risk factors. The problem is that high cholesterol has no symptoms. You cannot feel it. The only way to know your numbers is to test them.

A lipid panel is the standard blood test for measuring cholesterol and related fats in your blood. Here is what it measures, what the numbers mean, and what to do if yours are outside the healthy range.

What Does a Lipid Panel Measure?

A standard lipid panel includes four values:

Total Cholesterol

The sum of all cholesterol in your blood. While useful as a quick overview, total cholesterol alone is not a reliable predictor of cardiovascular risk — the breakdown between LDL and HDL matters far more.

Desirable: Below 200 mg/dL Borderline high: 200–239 mg/dL High: 240 mg/dL or above

LDL Cholesterol (Low-Density Lipoprotein)

Often called "bad" cholesterol, LDL carries cholesterol from the liver to the rest of the body. When LDL levels are high, excess cholesterol can deposit in artery walls, forming plaques that narrow and harden arteries — a process called atherosclerosis.

LDL is the primary target of cholesterol-lowering treatment.

Optimal: Below 100 mg/dL Near optimal: 100–129 mg/dL Borderline high: 130–159 mg/dL High: 160–189 mg/dL Very high: 190 mg/dL or above

For people with existing heart disease or diabetes, the target is typically below 70 mg/dL.

HDL Cholesterol (High-Density Lipoprotein)

Often called "good" cholesterol, HDL carries cholesterol away from the arteries and back to the liver for removal. Higher HDL is protective — it is one of the few lab values where a higher number is better.

Low (increased risk): Below 40 mg/dL (men), below 50 mg/dL (women) Acceptable: 40–59 mg/dL Protective: 60 mg/dL or above

Triglycerides

Triglycerides are a type of fat stored in fat cells and circulating in the blood. They come from calories you eat that are not immediately used for energy. High triglycerides are associated with cardiovascular risk, especially when combined with low HDL.

Normal: Below 150 mg/dL Borderline high: 150–199 mg/dL High: 200–499 mg/dL Very high: 500 mg/dL or above

Non-HDL Cholesterol

Some labs also report non-HDL cholesterol (total cholesterol minus HDL). This captures all the atherogenic (plaque-forming) lipoproteins — LDL plus VLDL and IDL — and is considered a more complete cardiovascular risk marker than LDL alone.

Optimal: Below 130 mg/dL

Does a Lipid Panel Require Fasting?

Yes — for the most accurate results, you should fast for 9–12 hours before a lipid panel. Eating, especially fatty foods, temporarily raises triglycerides and can affect the calculated LDL value.

Water is fine. Black coffee is generally acceptable but may be best avoided to be safe.

What Causes High Cholesterol?

High LDL and triglycerides can result from a combination of:

  • Diet — saturated fat, trans fat, and excess refined carbohydrates raise LDL and triglycerides
  • Genetics — familial hypercholesterolemia is a genetic condition that causes very high LDL regardless of diet
  • Inactivity — regular aerobic exercise raises HDL and lowers triglycerides
  • Obesity — excess body fat, especially abdominal fat, raises LDL and triglycerides and lowers HDL
  • Hypothyroidism — an underactive thyroid raises LDL
  • Type 2 diabetes — insulin resistance raises triglycerides and lowers HDL
  • Certain medications — corticosteroids, beta-blockers, and some diuretics can raise cholesterol

Who Should Get a Lipid Panel?

The American Heart Association recommends that all adults have their cholesterol checked at least every 4–6 years starting at age 20. More frequent testing is recommended if you have:

  • A family history of heart disease or high cholesterol
  • High blood pressure or diabetes
  • Obesity (BMI over 30)
  • A history of smoking
  • Previously abnormal cholesterol results
  • Are taking cholesterol-lowering medication (typically every 3–12 months)

Men over 45 and women over 55 are at higher cardiovascular risk and should test more frequently.

How to Improve Your Lipid Numbers

If your results are outside the healthy range, lifestyle changes can make a meaningful difference:

To lower LDL:

  • Reduce saturated fat (red meat, full-fat dairy, butter)
  • Eliminate trans fats (partially hydrogenated oils)
  • Increase soluble fiber (oats, beans, fruits, vegetables)
  • Add plant sterols (found in fortified foods)

To raise HDL:

  • Exercise regularly — aerobic exercise is the most effective lifestyle intervention for raising HDL
  • Quit smoking
  • Lose excess weight
  • Moderate alcohol consumption (1 drink/day for women, 2 for men) may raise HDL, though the cardiovascular benefits are debated

To lower triglycerides:

  • Reduce refined carbohydrates and added sugars
  • Limit alcohol
  • Increase omega-3 fatty acids (fatty fish, flaxseed, walnuts)
  • Lose excess weight

For people with significantly elevated LDL or high cardiovascular risk, statins and other medications are highly effective and well-tolerated.
